ADEA: Diabetes Referral Map in Primary Care and Between Primary and Tertiary Care

The Australian Diabetes Eduucators Association (ADEA) is holding GP focus group discussions to develop a framework that provides GPs and PNs involved in diabetes care with a suggested map for referral of persons with diabetes to:

  1. 1. Credentialed diabetes educators (CDEs) working in the primary sector.
  2. 2. Diabetes specialists (endocrinologists and CDEs) working in the tertiary care sector.

Discussions will pertain to:

  • Role of GPs in diabetes care—strengths and challenges
  • Existing referral patterns for people with diabetes
  • Trigger points for referral for people with diabetes
  • erspective on other professionals in diabetes care

Target Participants: General Practitioners
Tentative Dates: May 17, 2011 (Tuesday) or May 18, 2011 (Wednesday), Time: 6:30 pm – 8:00 pm
