The Victorian Department of Health, Immunisation Program has asked divisions to advise practices about :
10 dose vaccine pack problems
Infanrix hexa vaccine must be reconstituted by adding the entire contents of the syringe to the Hib vial and shaking until the pellet is completely dissolved.
Varilrix vaccine must be reconstituted by adding the entire contents of the syringe to the chickenpox vial and shaking until the pellet is completely dissolved.
The Immunisation Program has received several reports this week from immunisation providers who have missed reconstituting vaccines:
In these cases all infants who were given the vaccine from the 10 pack had to be recalled for re-administration of either Hiberix vaccine or chickenpox vaccine.
All the babies were recommended to have an additional Hib or chickenpox vaccine as it could not be determined which babies did indeed receive the reconstituted Hib and chickenpox pellets.
The errors have occurred when 10 dose vaccine packs of Infanrix hexa and Varilrix have been used. The pellets are located at one end of the packs and the syringes are at the other end of the pack.
