DoH: Palliative Care Clinical Network

An invitation is extended from Cancer and Palliative Care, Department of Health for applications for membership on the Palliative Care Clinical Network.

Please find attached the following three documents:

For electronic completion of the matrix:

  • Select ‘save as’ and save to a folder in your computer 
  • “Mouse click” boxes to select ‘Yes’ or ‘No’ options, or select ‘text form field’ to enter text 
  • ‘Save’ prior to closing document 
  • Attach completed matrix file to email or print hard copy

The due date for applications is Friday 9 July 2010.
