Life! FIRST VISITS SCHEME.
Do you want to prevent the onset of Type 2 Diabetes in your patients? Take part in the Life! First Visits Scheme.
A first visit provides the opportunity for a nurse or other health professional to individually discuss with eligible patients their risk of developing Type 2 diabetes, health concerns and motivation to change. During the first visit the Practice Nurse needs to confirm the patients risk of developing Type 2 diabetes, determine whether they prefer either a Life! course or Life! telephone health coaching, and refer them to their preferred mode of delivery.
The GP Clinic will be paid $83.45 per first visit. Clinics need to register to conduct first visits.
