Have these patients scored 12 or more on the AUSDRISK tool? Refer these patients to a local Lifestyle Modification Program today! Referrals are currently needed for programs at Carlton, Kensignton and Port Melbourne.
If you need help identifying these patients, Diabetes Australia – Victoria has some case funding available for practices to assist with recall and reminder letters, referral to a Lifestyle Modification Course as well as time for the doctors to complete pathology referrals.
