Prevention in General Practice Awards 2009 - Recognising commitment to chronic disease prevention

Are you leading the way in encouraging patients to address lifestyle risk factors? Are you building partnerships with your local community to do so? Do your primary prevention initiatives include the whole practice team? If you have answered yes to any of these questions, then the VGPPA wants to hear from you!

Individual practices are invited (regardless of size or structure) to highlight their work in chronic disease prevention by entering the Prevention in General Practice Awards 2009.

What sort of activities should be entered?
Primary prevention initiatives in general practice that address SNAPO lifestyle risk factors (smoking, poor nutrition, alcohol misuse, physical inactivity, overweight and obesity). Assessment criteria have an emphasis on effective systems, whole of practice approach, partnerships and sustainability.

Prizes
There are three prizes on offer:
Winner (general practice) - $3000
Highly commended (general practice) - $2000
Division of general practice prize - $1000

Prize money is intended for activities such as staff training, education and/or resource development to support further prevention activities.

Closing date: Entries for awards close 5pm, 18th September 2009
