RCH: Help your patient and the RCH Triage Staff

THREE REFERRING TIPS THAT HELP YOUR PATIENT AND HELP RCH TRIAGE STAFF:

SOCIAL AND PSYCHOLOGICAL HISTORY CAN BE VERY IMPORTANT: With patient consent, this helps assess urgency beyond the medical condition.

  • Ask your Practice Manager to install the RCH referral form to your Practice Software.
  • The form will auto-populate key demographics.
  • Avoid fax cover sheets – all contact details should be on the referral.
