RVEEH - Outpatient Booking Process

NEW REFERRAL REQUIREMENTS FOR RVEEH:

The Eye and Ear Hospital have been working on lean thinking processes within their Outpatient Booking Unit as part of the Redesigning Hospital Care initiative.

We have streamlined our process so that the minimum information required on a referral is as follows:

  • Legible
  • Patient’s Full Name
  • Date of Birth
  • Contact for patient: Address and/or Business hours phone number
  • Reason for referral
  • Relevant medical history to assist with triage.

 Electronic and printable referral templates are still available to be downloaded from http://www.eyeandear.org.au/healthprofs/gpinfo.asp
Please note that referrals cannot be processed without the above information.
