The MGPN geographic boundary includes several large public and private hospitals and several tertiary specialist centres. Through contact with the GP Liaison departments at each of these sites MGPN aims to enhance the channels of communication between General Practice and the hospital services available in this area.
This can include information about up coming education events conducted by the hospitals, feed back around referal and discharge documents, and service provision.
Do your patients know your details?
When your patients attend hospital, they are required to provide GP details, finding a patient’s GP & correct address can double the time taken to register a patient coming to hospital. When you are correctly identified as the patient’s GP you will receive OPD letters & discharge summaries.
Give your patients your business card and ask them to keep it with their Medicare Card. Or ask them to store the details in their mobile phone under GP. This will assist them in providing accurate information and prevent time consuming errors or follow up in the event that they attend hospital.
