Health Independence Programs

St Vincent’s and North Richmond Community Health have launched a combined initiative to improve the referral process for GPs to services which support the transition from hospital to home and prevent the need for a hospital presentation. This includes patients who are post-hospital discharge, sub-acute, ambulatory or have chronic or complex conditions.
