 LMP – Diabetes Prevention Program to cease November 1st, 2011

Please note, due to funding, the LMP program for preventing Type 2 Diabetes in 40-49 year olds has ceased on the 1st November, 2011. Referrals for 40-49 year olds can be made to the program up until the 1st November, 2011 after which time, referrals for 40-49 year olds can no longer be accepted to the program. Referrals for the Life! Program for prevention of Type 2 Diabetes for patients 50 years and over will continue after the 1st November, 2011. The Life! program explained

The Life! program is designed for those 50 years and over who are at high risk of developing Type 2 diabetes. Research has shown that by participating in a Life! program, Type 2 diabetes can be prevented in up to 58% of cases. The Life! program is a group education program aimed at assisting people to make healthy lifestyle changes to prevent the onset of Type 2 Diabetes. Patients 50 years and over who score greater than 12 on the Australian Diabetes Risk Tool are eligible to be referred to a fully funded Life! program.

The course involves six, 90 min sessions designed to improve nutrition and physical activity over an eight month period. Participants will be supported by trained health professionals, including a Dietician and Exercise Physiologist to make positive lifestyle changes.

Your Practice’s Role in the Life! Program

General practice plays a very important role in the Life! program by supporting and testing people identified as being at risk of diabetes. Advertisement of the programs and placement of risk tests in visible areas in your waiting room will encourage patients to self assess their risk of developing type 2 diabetes. After completing the Diabetes Risk Test, patients scoring over 7 are encouraged to visit their GP to have their score validated. All patients with a score over seven should then have diabetes excluded. Those patients that score over 12 or greater, who do not have diabetes will be eligible for a Life! course. GP’s are then required to refer their patient into a Life! course by completing the referral form and faxing to Megan at MGPN on 9438 0750.
